What is the minimum and maximum investment?

What are the returns?

Investors will receive a preferred 8% annual dividend paid in monthly payments and a 1.5x return at the time of refinance in 4 years. The internal rate of return is 17.6%



What is the hold period?

Investors will hold the property for 3-5 years before refinancing.

Dave Simmons


Dave was fundamental to the success of financing and development of more than $750 million in real estate transactions during his finance career. He matched many large developers with institutional equity. He has successfully implemented a value add strategy for over 60 syndicated real estate deals, even during the economic downturn of 2008.

Joe Heaney

Chief Strategy Officer

Served as CEO of 6 companies, with multiple experiences of start-up to $100m, in a wide variety of industries and different business situations. Experience includes: President of Cornnuts, Chairman of the board of Clearwave Energy Inc., President of Thoughtful Food Inc, Advisory Board of Crave Bakery, as well as several others. Graduate of Harvard Business School.

Anthony Barbato

Chief Executive Officer

Worked as a Realtor specializing in investment cash flow and distressed properties. Focused primarily on prospecting for and analyzing off market deals for real estate investors. Maintains a consistent 120 hour work week for Consolidated Carehomes Inc. Hosts weekly in person meetups to generate investor interest in projects. Business Management degree from Southern Illinois University.

Peer Services Inc. (PSI) is a firm specializing in the development, management and consultative services for the senior living, skilled nursing, and rehabilitation center industries. PSI is a service oriented business in the health care and senior living sector. PSI has experienced tremendous growth in the past four years as a result of strong leadership, creative problem solving and the relentless pursuit of creating value for our clients. PSI is composed of retirement community administrators who have maintained positive net income with an occupancy rate of 90 percent or more for the past 23 years. PSI has received high scores in resident satisfaction surveys and have earned Five Star ratings in state and federal inspections.

David K. Reimer

President and Chief Executive Officer

Over 35 years experience in senior and long term health care fields, most of those years providing comprehensive leadership in a Continuing Care Retirement Community. In addition, experience in renovation/addition to existing retirement community campuses, designing and construction supervision for new retirement communities, opening new communities, staffing capital campaigns, securing long term financing, starting unique programs and services intended to provide options to seniors not living on a retirement community campus, lobbying state and federal leaders, providing leadership to trade association groups, creating and updating services which exhibit successful outcomes and profitability.

Dale Torry, J.D.

Managing Director

Entrepreneurial leader with experience in organizational growth, turn around success and start-up leadership. A life-long learner. Experienced in board development and board governance. Has lead multiple strategic processes with staff, board and key stakeholders. Leading the development of approximately 300 units of senior living while repositioning client into a CCRC/LifePlan community in Southern California. Developing 287 beds of skilled nursing in Southern California to address bed shortage and mental health needs. Leading development discussions for clients with prospective projects ranging from $5 million to $75 million.

Darren L. Trisel

Vice President of Operations

Ten-years as a Senior Healthcare Executive with credentials to include; new business formation – court monitor/expert witness/consulting, financial and clinical turnaround, multi-facility, budget formulation, profit and loss administration, zero deficiency regulatory compliance, multi-line insurance and risk management, and creation of nationally recognized person-centered care best practice models. And, Darren held Board and Committee seats for non-profit healthcare associations including; public speaking, delegate responsibilities, and lobbying efforts at a national level.

John Ralosky


With over 30 years of experience in senior housing, John has served as Chief Operating Officer for Solvere Senior Living, Senior Vice President of Operations for SeniorBridge Home Health Care, Benchmark Assisted Living and Atria Senior Living.

Currently John is partnered in two ventures to consult to senior housing and source equity for senior housing projects. John has turned around troubled senior living communities and healthcare providers in over 22 states. Career highlights include the turn-around of three failing CCRC’s in Indiana, Ohio and Massachusetts; two of which represented a two cents per share operating loss for the then publicly traded Manor Care.

At Benchmark, John’s leadership was instrumental in bringing the portfolio occupancy to 95% from a stalled 60% rate. John led the management team of Atria’s Northeast Division with 26 properties and became the first division in the company to achieve over 90% occupancy Additionally, in most of his positions, he restructured middle management positions to improve accountability and results, lowered operating costs and improved employee retention.

With over 25+ years in the senior living industry, Andrew has a proven track record in building teams, creating a collaborative vision and plan to achieve consistent positive outcomes…exceeding financial targets along with exceptional employee and customer satisfaction. With leadership roles in Operations, Sales and Marketing, his focus is on the customer and the individuals and teams who support them.

Andrew has served as Senior Vice President for Carlton Senior Living as well as Divisional Director of Operations for Westmont Living, Regional Sales for Atria Senior Living and Benchmark Assisted Living, Director of Operations for Senior Living Residences, working in 7 states with significant experience in start-up and turn-around of challenged, underperforming projects.

Serving thousands of residents in his career and a track record of increasing the occupancy, net operating income and resident satisfaction rates. This strength in leadership and empowerment to those on the team provides consistent results on so many levels…decreased turnover, reduced recruitment costs, retained institutional knowledge and again, continuity of productivity. Strategically and thoughtfully looking at needs in the market, assessing the opportunities and putting a plan in place to get there, again with buy-in from key-stakeholders and collaborative efforts with the team to achieve results.
